What the service does
Advance Care Planning Australia helps people plan for future health care decisions. It provides information, resources and guidance to help people record their wishes and nominate decision makers on their behalf if they are unable to make or communicate decisions in the future. People can also access support on how to have these conversations with family, healthcare professionals and aged care providers. The service is available across Australia.
